What is cash-on-cash return and how does it differ from yield?

Yield measures income against property value. Cash-on-cash return measures income (after mortgage costs) against the cash you have actually invested, including deposit, legal fees, stamp duty, and any refurbishment. It is the more relevant measure for leveraged buy-to-let investors.
